Please register your player before July 7th. All players must be registered before the roster process can begin. For questions on the ATSC fee structure and payment deadlines, please see Steven Lent’s letter on ATSC Fee Structure and Registration FALL 2008.
Also, please get the following to your managers as soon as possible. The deadline for the forms is not July 7, but the sooner the better:
- Medical Release—return after it has been notarized. Everyone will need get this form notarized on your own. Don’t sign until you are in the presence of the Notary. The easiest way to do this is to go to a bank. Make sure you have ID with you.
- Code of Conduct: Player and Parents—both parents and players need to read and sign.
- One 1" x 1 1/2" color picture for your player (school picture size is good).
- Child ID—Original birth certificate or passport (not hospital birth certificate). These will be kept until after the registration process is completed. The document must be an official, government issued document. A Child ID Card from the DMV can also be used,... and in general, this is a good form of ID to have for kids.
