Please complete and sign the following forms and return to Mario or Marc as soon as possible:

Code of Conduct  - to be signed by Player and Parents

Medical Consent Form - Notarized.  Must be signed in presence of a Notary Public.  Most banks provide this service.

For players new to Travel Soccer, we will need a notarized birth certificate or passport (can be expired).  Originals only - these will be returned once the registration process is complete.

If possible please bring these items to the practice on Tuesday, July 15 @ Gunston.
